The first case of appendicitis is a harrowing tale of the only medically trained person on the whole continent having to cut into himself or face certain death.
The second one just did it for the lulz.
Second one seemed more like a flex than for the lulz, but yeah. Definitely a “because I thought it’d be cool”, which is a wild reason for self-surgery
